If you have received reimbursement from the Kentucky Fire Commission for occupational stress injury treatment that was initially paid for by The Chip Terry Fund, you can conveniently submit your reimbursement using the secure form below.
Please use this form only if you are repaying The Chip Terry Fund after receiving your Kentucky Fire Commission reimbursement.
If you have received financial assistance from The Chip Terry Fund and have not yet applied for reimbursement through the Kentucky Fire Commission, you may be eligible to recover the cost of your approved treatment.
